In Person Events

RELEASE/WAIVER: I hereby agree to indemnify and hold harmless HeavenScentLLs and its employees at any locations in person events are held (addresses are inputed on physical waivers at the time of events), against all claims for personal injuries or damages of any kind arising from participation in any Candle Making workshop, specialty classes, and their resulting products.
*PHOTO CONSENT: By your signature, you agree that HeavenScentLls may use the below named participants photograph in the routine promotion of its activities.
SUBSEQUENT VISITS: This waiver shall be kept on file and adhered to for both initial and subsequent visits to the workshop by the participant.
You may revoke this waiver at any time by sending a statement via certified mail to: HeavenScentLLs (address will be provided).
I fully understand and acknowledge that the workshops offered at HeavenScentLls may involve inherent risks, dangers and hazards which may result in injury or illness. All Workshops offer a No Refund Policy.
I hereby voluntarily agree to release, waive, discharge, hold harmless, defend, and indemnify HeavenScentLls its managers, members, and employees from all claims, actions or losses for bodily injury, property damage, wrongful death, or otherwise, which may arise from my participation in any workshops or classes, resulting products from workshops or classes, or from use of equipment provided by HeavenScentLls.
I accept full liability for creations made by the participant in the workshop.
